The OSUMyChart app is designed to help patients and visitors find their way around the OSU Wexner medical center. It includes a map with directions, information about parking garages and visitor lot locations, as well as links to important hospital departments like patient relations and emergency services. You can also use the app to call for an ambulance, request access to your personal records, or search for some of our most popular resources like “What’s New at Wexner Medical Center” or “Patient Rights and OSUMyChart Login Portal.”

OSUMyChart is a free, secure patient portal that allows you to view, download or share your medical records. OSU my chart also provides the ability for patients to schedule appointments and refill prescriptions online with an affiliated pharmacy. To Ohio State University mychart login to the system, you will need your Ohio State University ID (ex: jane@osu.edu) password, which should be changed regularly! The server is located here in Ohio so if it’s not working try logging in from there instead of on the website – this could give you an error message about being offline when they’re actually just experiencing some technical difficulties with their servers. View your personal health information such as lab results and demographic information that are stored at The Ohio State University Wexner Medical Center (WMC)

Make updates to personal information including weight, height, medications, or medical conditions like diabetes or asthma. You will also find in-depth procedures we have performed including CT scans and MRI’s. Anyone who has ever been treated at WMC may view their records through OSUMy Chart but only authorized people can edit them.

OSUMYCART Sign in & Register

If you are an Ohio State University student and would like to register for the service, select Register Now. You will be prompted to enter your information into a form that fields including name, phone number, address of permanent residence (this is where we send notifications if there is an emergency), date of birth, signature on the registration page. Once submitted you have successfully registered and should now log in with your new username and password. Mychart Dupage 

Once logged in as a current OSUMyChart user:

Select My Chart from the menu bar at the top left corner of this screen or go directly here: mychart-osu-.edu/OSUMYCART_USERNAME.

This will take you to your personal medical chart which provides summary health information.

The chart includes your immunizations, lab results, allergies, and other medications you have been prescribed at Ohio State University Wexner Medical Center as well as any visits to the OSU Health Center or emergency room that are related to medical care.

Select an item from this list for more detailed information about a specific test result: Blood Pressure Results (Many of these can be found in OSUMyChart lab’s section).

To make changes to items on your personal OSUMyChart account click “Make Changes” on the bottom left side of the page once OSUMyChart Login. This will take you to another screen where you can enter new medical conditions and prescriptions among others things; after changing those values select Save Changes at the bottom right of the page.

As a student at Ohio State University, you have access to OSUMyChart!

Your chart includes your immunizations, lab results, allergies and other medications that were prescribed for you by Ohio State Wexner Medical Center as well as any visits to the university or emergency room related to medical care.

You can also make changes like adding new conditions or prescriptions in this section of your account too. Be sure to click “Save Changes” when finished so they are saved permanently on your OSUMYCART profile. What is OSU Wexner Medical Center?

OSU Wexner Medical Center includes The Ohio State University Wexner Medical Center, The Ohio State University Hospitals East Campus, and Nationwide Children’s Hospital; all three campuses offer medical services to patients from across central and southern Ohio.
